Required Documentation

Important! In order to participate in any 金多宝心水论坛 athletic activity, the following information must be completed and turned in to the school's athletic department via :

1. A "Opioid Education: The Game You Can't Win" and "Barrow Brainbook" must be on file at the school. These courses must be completed prior to any participation, and need to be completed only once during the school year by the student-athlete. 

2. A copy of the participant's birth certificate needs to be on file at the school.

3. Your child's annual physical evaluation. Parents must use the for their student-athlete's physical.  Physicals received after March 1, 2024, are acceptable for the 2024-25 school year. Please download, print, and take the physical evaluation form to your child's doctor to fill out.

4. Complete all other forms on the website.
